New Patients

Register as New Patient

Patients are registered with the practice rather than an individual doctor. To Register with the practice you will be asked to complete these forms.

HS200 - to be completed by anyone joining the practice from another practice WITHIN Northern Ireland (HS200 Form) https://hscbusiness.hscni.net/services/1815.htm 

HSCRI - to be completed by anyone joining the practice from OUTSIDE Northern Ireland (HSCR1 Form) https://hscbusiness.hscni.net/services/1814.htm 

You will also need to provide:

  • Photographic ID
  • Proof of Address
  • BSO may also request further information
  • Confirmation from your previous GP of any medication you are currently being prescribed

Asylum Seekers must provide a BAIL201 Card and an Application Registration Card to register with the Practice.

Medication 

NO MEDICATION will be prescribed until you have completed the New Patient Registration. Prescribing any medication must be through a doctor. You must speak to a Doctor either over the phone or face to face. This practice does not prescribe sleeping tablets, anxiety tablets and strong painkillers unless recommend by the GP. If you are currently on sleeping tablets or anxiety tablets these will be reviewed, reduced and eventually stopped in line with best practice as most of these drugs are not for long term use. Your previous GP is responsible for your medication requests until you have been accepted onto our Practice List.

Disabled Patient Facilities

Disabled patient facilities are available at this practice.

(Car park space, ramp, toilet etc).

If you have hearing loss or deafness the Practice has a range of methods for you to contact the surgery.

  1. Email - please email the Practice at prescriptions1.Z00061@gp.hscni.net with your queries.
  2. Text messaging - we can provide a two-way text messaging service for a GP to communicate with you.
  3. The British Sign Language (BSL) online - this can be used when you attend the surgery. If possible please email us with your preferred options to record in your notes.

Updating Your Details

In case we have to contact you urgently, please notify us immediately if your address or telephone number changes.

You can update your details by registering for Patient Access and updating any changes to your personal records via the Patient Access homepage.
